*You must bring:
~ Confirmation Message Page,
~ Physical Forms:
~ Physical Form (must be stamped and dated 6/5/18 or later)
~ History Form
~ Parent Medical Statement & Emergency Form
~ Copy of insurance card (must have student name)
~ Transportation form (with all signatures)
Drop off all paper work to the front office or athletic director office in order to get cleared.
*Please note that you do NOT need to come by during office hours - you can leave your paperwork in the mailbox of the athletic director and you will receive a notification when your student is cleared.
